nipple pain

hi denise,
i have a mother who is to deliver next month. she has complains of inability to breastfeed her first child due to severe pain in her nipples. she also experiences some pain during every menses.i examined her breast yesterday, and found her nipples fissured (the nipple tip was inturned, but it was not inverted nipple).when i did nipple protrusion test she experienced severe pain. what can i do for her , so that she can breastfeed this time? have you seen cases like this?

I have heard of similar cases but not had one myself. How does she react with a breast pump - guess you'll have to wait until she delivers though to find out.
A nipple shield may help, but you'll probably need to express milk too in the early days to ensure the baby is getting something - in my experience until the mother has a good breastmilk supply nipple shields restrict too much of the milk that baby needs.
A wonder if a mild analgesic could help - maybe she'd only need to use them for a week until her nipples are used to being drawn out during breastfeeds. It's worth a go - still better for baby than infant formula.
Also, I hesitate to suggest this in an open forum, but her partner may be able to assist until such time as the baby is born....

do i need to teach her some nipple stretching exercises now, so that she has less pain later? has anybody managed cases like this?
